Ordering Oregon POLST Forms and Materials
The POLST order form is for Health Care Professionals and organizations only. Individuals seeking to obtain a POLST Form may do so through their health care professional.
Need a Spanish POLST form?
The POLST form is read by emergency medical personnel, nurses, and physicians who may speak only English. In order for POLST Forms to be portable, they must be in English. However, a Spanish form is available to help explain the orders to Spanish speaking persons, but the only form that is valid to be used in the field is in English version.
The Oregon POLST Form is updated approximately every two to three years. As of October 1, 2014, version 9 of the Oregon POLST Form is effective. Prior versions of the POLST Form remain valid. If the patient's wishes have not changed, a new POLST Form does not need to be completed.
The biggest changes on the 2014 version are in sections D and F which now require attestation by the signer that a conversation was held with the patient or surrogate, as well as to specify with whom the conversation occurred. Increased documentation and verification of the conversation has become the standard nationally.
